After defeating Mahoraga, Gojo thought that Sukuna no longer had any way to circumvent his Infinity. I think the usual Gojo would have sensed that something was wrong and could have avoided a fatal injury.

These words confirm what some suspected: Gojo lost because he lowered his guard.

A human error... and a tragic one

This isn't the first time he's been overconfident. During his youth, Toji Fushiguro managed to seriously injure him. At the time, Gojo was exhausted after several days without sleep, which cost Riko Amanai her life. It was this event that subsequently triggered Geto Suguru's betrayal.
